Congratulations on completing the annual Popcorn Fundraiser!

There are 2 ways to register your Scouts for the "Q For a Day" experience and the "Fill-It Up" program.

1. The preferred method is to submit your information on an Excel worksheet to cmanteau@cavs.com.

If you fill out the worksheet, please try to fill in as many of the lines as possible.

2. If you do not have access to Excel, or, if you only have a small number of Scouts, you may register them by completing the online survey at the bottom of this page. This will allow you to enter up to 10 Scouts at a time.

Please fill out this Registration Form as completely and accurately as possible for all of the Scouts in your unit that contributed to this year's popcorn fundraiser to ensure they receive the proper incentives.

As a reminder, Scouts that sold to a minimum of 3 customers will be invited to the special "Q For a Day" event on Jan. 31, 2009. Scouts that "Filled-Up" their order forms will also receive an invite to the "Q For a Day" event and special offers from the Cleveland Cavaliers, Cleveland Indians, and Cleveland Gladiators.

POPCORN SELLER INFORMATION

Please fill out the following items for each Scout in your unit that sold popcorn to at least three (3) customers* (Name and Address of each Scout is required). When finished entering the information, please proceed to the end of the form.

You may enter up to ten (10) Scouts on this form. If more than 10 Scouts from your unit qualified, please enter your first 10 Scouts and complete the form. Then, log back in as many times as necessary to enter the remaining Scouts.

NOTE- To save time, you may also submit your information on an excel spreadsheet to cmanteau@cavs.com. Please be sure to include answers to all of the questions on this survey (Unit name & number, Council, Leader information, individual Scout information, popcorn sales numbers, etc.).

(*The sales numbers will be cross-checked with the Trail's End & Scout Council databases. A Scout must have a minimum of 3 customers to receive the Q For a Day invitiation and must meet the council's "Fill It Up" criteria to receive the Cavaliers, Indians, and Gladiators ticket offers.)
